Request Your Academic Records
Request official transcripts from all institutions you’ve enrolled since last at Southwest. To be considered official, the transcript must be issued to Southwest Tennessee CC and must come directly from the institution. Transcripts are needed even if credits from one college appears on another transcript, and even if no credits were earned. Failure to disclose all college enrollments could result in your status being re-evaluated.
Prior to 2011, Southwest did not maintain documents submitted over five years. If it has been five years you may be asked to resubmit transcripts.
Students unable to obtain official transcripts due to indebtedness are not eligible to enroll at Southwest as degree-seeking. If you have an outstanding balance at another Tennessee institution, check to see if the school participates in the Student Promise-to-Pay program which may allow you to make pay arrangements and obtain your official transcript. (Contact the Bursar’s office for details).
Note: if you previously attended a Tennessee institution that is now closed, request a transcript through Tennessee Higher Education Commission (THEC) using the closed-institution transcript request form. (Note: do not send this form to Southwest.)
Immunization Health History Form (if you are under 18 years-old)
To be considered for aid, all students must be admitted to Southwest as degree-seeking. Complete the "Free Application for Federal Student Aid (FAFSA)." Southwest's school code is 010439. For more information, contact Financial Aid. Note: during the evaluation of your financial aid status, the financial aid office may require additional official college transcripts be added to your admissions file.
Academic advising is required for all students before being allowed to register. Once you have been readmitted you should set up an advising appointment. For more information, contact Advising.
